Responsibilities

  • Be the first point of contact for patients, families, and referral sources—providing compassionate support and ensuring a seamless experience from intake through care delivery.
  • Manage referrals and authorizations by triaging calls, verifying insurance, processing documentation, and expediting approvals to ensure patients receive timely care.
  • Support care coordination by maintaining accurate records, monitoring compliance, and partnering with internal teams and community providers for smooth transitions and effective communication.
  • Leverage technology and reporting as an EMR superuser—generating reports, ensuring accurate data entry, and recommending process improvements.
  • Contribute to program success through backup support for leadership, new market launches, and ongoing quality and performance initiatives.

Qualifications

  • HSD or equivalent.
  • Minimum of two years of experience in home care, hospice, or palliative care (preferred).
  • Proficiency in computer systems, including electronic medical records (EMR), with accurate typing and data entry skills.
  • Demonstrated ability to establish and maintain professional relationships with clinical staff, referral sources, and community partners.
  • Proven ability to work both independently and as part of a team; able to prioritize, multitask, and make sound decisions.
  • Strong knowledge of billing, credentialing, and insurance processes (preferred).
  • Excellent verbal, written, and public speaking communication skills.
  • Licensed driver with a reliable, insured automobile that meets state and organizational requirements.
  • Possess and maintains current CPR certification if required by state.

$35.00 - $37.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
