Continuum Hospice is currently seeking a Per Diem On-Call Registered Nurse (RN) to join our team, providing exceptional care and support to hospice patients and their families during their most critical moments. As an On-Call RN, you will play a vital role in identifying patient and family needs, coordinating care, and offering preventative measures and compassionate support.   Schedule: This is a Triage position which will be from 8:00am-8:00pm every other weekend (Saturday and Sunday only)
